For employees and students

Complete an "Employer's Report of Occupational Injury or Disease" form
Submit to Juanita Crook in Labor Relations

For visitors and students

Complete a "First Report of Injury" form
Submit to OHS

Investigate (by Supervisor)

Complete "Injury/Illness Loss Investigation Report" form
Submit to OHS
