Injury/Illness Reporting/Investigation ProgramInjury/Illness reporting and investigation is the responsibility
of departmental supervisors and departmental safety committees.
Occupational Health and Safety coordinates
all injury/illness investigations and receives originals of all employee,
student, and visitor investigation report forms. Labor Relations is
responsible for employee workers compensation activities. Employee
injury/illness reports and copies of
investigation report forms must be submitted to Labor Relations. Student and visitor
injury/illness reports and investigation report forms must be submitted to the
Department of Occupational Health and Safety.
